Authorization Agreement

An Authorization Agreement form for approval of monthly contributions from your credit/debit card account can be downloaded on this page with Acrobat Adobe Reader. If you do not have Reader click here for free download. After completing the form, you can either mail or fax it back to us. Please note that monthly contributions will be debited from your account until we have received written notice of termination.

Internet

Using the Internet to make contributions is fast and easy. Plus, it allows you to control how often you give. If you’d like to make an online donation, click here.

U.S. Missions accepts Visa, MasterCard and Discover.

A receipt will be mailed for your records, and the gift will be listed on your monthly credit card statement.

Monthly contributions will be charged to credit/debit card accounts on the 20th of each month.
