- If you use Google Chrome, you may get a "Please wait" error message. Click "Open the file with a different viewer" and choose Adobe Reader.
MAC USER INSTRUCTIONS
- If you do not have the Adobe Reader application for Mac, download the latest Adobe reader for Mac. Follow the on screen instructions to install the program.
- Do not open the form. Instead, save the PDF file to your desktop.
- Right click (or CTRL + left click) on the form saved to the desktop, choose Open With, then Other. Find Adobe Reader on the list of available programs. Check the box that says Always Open With and then press Open.
- The form will open in Adobe Reader, and all future PDFs will now open in Adobe Reader by default.
Fill out the form, save again, then manually attach to an email. Email the form to AGUSM Accounting.
NOTE: Fillable PDF files do not always work well on mobile devices.
